Together with our Health at Home™ partner, this group directs and monitors clinical quality, regulatory compliance, financial stewardship and quality assurance, while ensuring attentiveness to the health and wellness needs of seniors and their families.

Over the years, we’ve created several programs designed to enhance our residents’ health care experience. Our newest program, Health and Wellness Navigator™, focuses on three key areas.
Coordinating external health care services not typically available the community. This can include coordination of physician appointments, laboratory, radiology, pharmacy, home health, and transportation services.
Coordinating health care services within the community including nursing, clinic visits, and level of care changes.
Coordinating health and wellness programming of interest to residents for both body and brain health.
We created two programs designed to enhance our residents’ health care experience. First, LCS CARES™ – a resident-centered culture in skilled nursing and assisted living. In fact, CARES stands for Centered Around Resident-Empowered Services, and it fosters an atmosphere that supports independence, personalization and choice for all residents.
Second, Heartfelt CONNECTIONS – A Memory Care Program® is an individualized program that reduces anxiety and increases safety and well-being by focusing on social interactions and familiar daily activities. The person-centered, activity focused continuum-of-care philosophy is grounded in the belief that the abilities that remain are far more important that what is lost. Our memory care program builds on the individual, one success at a time.
As a result of our proven processes and procedures, nearly 90% of health centers managed by Life Care Services, An LCS Company, earned a 4- or 5- star rating. Only 10% of all providers in an entire state can receive a 5-star rating from the Centers for Medicare and Medicaid Services (CMS).
